Probably done at different times. Perhaps suitable cutaways not done at
the initial shoot - or even the pic changed after that for whatever
reason.

Continuity isn't his/her job. And could well have been a different one

More likely cost cutting.

If it was done at a different time the correct way is to have a viewing
copy of the original to check for such things. But that costs.
