Pat's Note; I won't be taking the PSIS out of this initiative. http://www.thefirmmagazine.com/news/1046/Crown_Office_steps_in_and_orders_Fatal_Accident_I nquiry.html NEWS 03 Sep 2008 Crown Office steps in and orders Fatal Accident Inquiry The Crown Office has announced that a Fatal Accident Inquiry is to be held into the death of 66 year old Margaret Rowan, who died following an outbreak of E-coli 0157 in the Renfrewshire area. It was reported last year that environmental health officers had drawn attention to more than health related 20 issues at the two Morrisons supermarket branches in Paisley during the past three years, which were highlighted as a possible source for the outbreak in which Margaret Rowan, 66 died and eight others were taken ill. Sliced cold meats from the delicatessen counters were believed to have been the source of the infection, and were withdrawn. The decision to hold an FAI follows "extensive investigations" by the Outbreak Control Team and a joint investigation by the police and the Environmental Health Authorities in Renfrewshire under the direction of the Procurator Fiscal.
